Apply Online & Required Forms
If you do not yet hold the endorsement and wish to apply to the DMV to obtain a "V" endorsement, you will need the following forms.
The process once submitted to the DMV typically takes 6–8 weeks. Fingerprints will be obtained and submitted to the DMV in addition to the D.O.T. physical and consent forms.
